Description

Singulair® Soft manufactured by Merck Sharp & Dohme. The medication is indicated for the maintenance treatment of asthma as add-on therapy in those patients with mild to moderate persistent asthma who are inadequately controlled on inhaled corticosteroids. The medicine is also used in the treatment of asthma in which the predominant component is exercise-induced bronchoconstriction. Singulair® Soft helps prevent the wheezing and shortness of breath caused by asthma and decrease the number of asthma attacks, but not to treat its acute attack. Singulair® Soft’s active ingredient is Montelukast that belongs to the class of medicines known as oral leukotriene receptor antagonists. It works by blocking certain natural substances (leukotrienes) that may cause or worsen asthma and allergies. The use of the drug improves breathing and reduces inflammation in the airways.

Indications:

- maintenance treatment of asthma;

- prevention of exercise-induced bronchoconstriction (EIB);

- seasonal and perennial allergic rhinitis.

MOA (mechanism of action):

Montelukast contained in Singulair® Soft is a leukotriene receptor antagonist (LTRA) used for the maintenance treatment of asthma and to relieve symptoms of seasonal and perennial allergies. It works by blocking substances in the lungs called leukotrienes that cause narrowing and swelling of airways. Blocking leukotrienes improves asthma symptoms and helps prevent asthma attacks. This reduces the bronchoconstriction otherwise caused by the leukotrienes and results in less inflammation. Because of its method of operation, the medicine is not useful for the treatment of acute asthma attacks and does not interact with other allergy medications such as theophylline.

Dosage

The medication is be taken orally once a day with or without food. The tablets should be chewed.

Asthma:

- Adults (15 years of age and older): 10 mg once a day.

- Children (6 to 14 years of age): 5 mg once a day.

- Children (2 to 5 years of age): 4 mg once a day.

Singulair® Soft should be taken once a day in the evening.

Exercise-Induced Bronchoconstriction (EIB):

- Adults (15 years of age and older): 10 mg once a day.

- Children (6 to 14 years of age): 5 mg once a day.

- Children (2 to 5 years of age): 4 mg once a day.

For prevention of EIB, Singulair® Soft should be taken at least 2 hours before exercise. Do not take more than one dose in 24 hours.

Allergic Rhinitis:

- Adults (15 years of age and older): 10 mg once a day.

- Children (6 to 14 years of age): 5 mg once a day.

- Children (2 to 5 years of age): 4 mg once a day.

For allergic rhinitis, Singulair® Soft should be taken once a day.

Patients with both asthma and allergic rhinitis should take only one Singulair® Soft dose daily in the evening.

Missing a dose

If you miss a dose of Singulair® Soft you should take it as soon as you remember. If it is the time for the next dose you should continue your regular dosing schedule. Do not exceed the doctor’s recommended dose.

Overdose

If an overdose occurs and you are feeling unwell, you should seek emergency medical attention or contact your healthcare provider immediately.

Storage

Store at room temperature between 15 and 30 degrees C (59 and 86 degrees F) and away from excess moisture and heat. Keep your tablets in the blister pack until it is time to take them. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

Side effects

The most common side effects associated with Singulair® Soft are:

  • - bed-wetting or loss of bladder control in children
  • Less common side effects include:

  • - shortness of breath or troubled breathing

Side effects occurrence does not only depend on the medication you are taking, but also on your overall health and other factors.

Contraindications

Do not take the drug if you are allergic to Montelukast or any ingredients of this medication.

Do not use Singulair® Soft to treat an acute asthma attack, including Status Asthmaticus.

Do not use the drug if you suffer from severe kidney failure.

Do not combine the medication with drugs which induce the CYP450 enzyme system (phenobarbital and rifampin), as they may decrease the effects of Montelukast.

Do not use the medication if you have a phenylketonuria, as Singulair® Soft contains phenylalanine.

o not use the Montelukast chewable tablets during pregnancy or while breastfeeding without consulting your doctor.

Do not give the drug to children under 2 years of age.

If you are taking the drug once a day on a regular basis, you should not take an additional dose before exercising. In the treatment of allergic rhinitis, the tablets may be taken at any time of day. Do not exceed a doctor’s recommended dose.
